Pulmonary

• dyspnea
• physical signs : tachypnea, tachycardia, 호흡보조근 사용, hypercapnia로 depressed consciousness, paradoxical abd wall movement
• wheezing, dyspnea on exertion, orthopnea, paroxysmal nocturnal dyspnea, leg edema
• hypoxemia = 혈액 내 산소 저하, hypoxia = 조직 내 산소 저하
• HF VS pulmonary causes 구분하는 것이 중요

• BNP = increase by ventricular overload

  HF, MI, sepsis, pul embolism COPD 처럼 폐 문제여도 pul HTN → cor pulmonale로 심장에 무리 생기는 경우에도 상승

• treatment goal = PaO2 >60, SaO2>90

• ARDS (acute respiratory distress SD)

• high flow → intubation 고려

• atrovent, mucomyst nebulizer, cortisol, anti

• hypercapnia

• PaCO2>45, RR 감소
• intracranial pressure 증가, headache, confusion, lethargy, seizure, coma, cardiovascular collapse
• PaCO2 10 = HCO3 1 증가해 compensate

• treatment = RR, volume 증가시키기 / O2는 sat 88-92 target으로 과하게 주지는 않기(RR 감소해버림) = high flow 달기, 60L 로 volume만 최대치로, 60L/40% / nasal prong 1-2L

• hyperventilation

• 과호흡에 의해 PCO2 감소, 손발 저림 증상 발생 가능

• cyanosis

• central = mucous menb and tongue - by hypoxemia, pul problem, abnormal Hb → sat 감소(abnormal Hb 은 normal)
• peripheral = finger or extremities - by vasoconstriction, decreased circulation → sat normal

• O2 apply → 효과 없는 경우, impaired circulation(shock), abnormal Hb, pseudocyasnosis(by drug) 의미

• wheezing VS stridor in dyspnea

wheezing 이 들리는 dyspnea 환자의 경우

→ pul hx. (COPD, asthma) 있다 = nebulizer(SABA)로 기관지 확장 시켜주기, cortisol** 로 항염증 ** (respiratory acidosis로 CO2 retention 있으면 high volume O2 로 빼주기)

• wheezing 없으면, COPD(steroid) asthma(SABA) exacerbation 아닌 상태

→ cardiogenic 으로 HTN, HF 있다 = Lasix

• Pul. edema (급성 폐부종) - 주로 lt. heart failure 로 발생, 폐 조직 내부에의 부종 생기는 심인성! → 두 가지 원인 구분 위해, xray pro-BNP echo 등으로의 평가 중요
• 앉은 자세 유지, 고농도 O2 제공은 공통으로
• normal CXR 는 아래쪽 vessel 만 보임 → 윗쪽 vessel 들도 보임(sunbursting sign 으로 손 흔드는 것처럼) → alveolar phase 에는 전부 다 뿌옇게

• 심인성인 경우 약물 - 혈압 감소 및 체액 배출 MAIL DONi**

  D5W, Lasix (furosemide 이뇨제) 1, Nitroglycerin Nitroprusside (혈관 확장으로 preload, afterload 감소해 심부담감 감소), Dobutamine (inotrope, 심수축력 높여줌), Morphine** (불안감 낮추고, 약간의 혈관 확장 효과) < 만약 심인성인데 혈압이 낮다 ? → lasix 보다 dobutamine 을 먼저 주기!!! >

• 비심인성인 경우 (ARDS, 패혈증, 흡인성폐렴 등 폐 자체의 손상)

  원인치료 - 항생제 (폐렴, 패혈증)

• Pleural effusion - 폐 바깥 공간인 흉막에 물 차기

• CHF 로 심인성인 경우(transudative effusion 누출성 흉수) : lasix
• CO 감소로 폐정맥 압력 증가해 혈관 속 맑은 액체가 폐 조직과 흉막강으로 새어나옴
• 폐렴, 결핵, 패색전증, 악성 흉수(exudative fuusion 삼출성 흉수/ptn 많음) : 흉강 천자술 및 원인 치료
• 염증 반응으로 혈관 투과성 증가하여 염증성 액체가 새어나옴
• ptn 높고 glucose 낮은 경우, **ADA >40 이면 결핵
• 흉관 삽관 고려 = pH <7.2 glucose <20 LDH>1000 empyema**

• Pneumothorax

• 기흉 산소 흡수 도움되게 O2 nasal 로 4L(3-10L) 정도 주기, ctd insertion 위해 cs 연결

• PTE ** → 순환기내과, DVT 외과**

• Mcconnell’s sign ** = echo 상, RV enlargement, akinesia of mid free wall, apex of RV 부위만 팔딱팔딱 **= D shape 은 pul HTN 에서도 가능하며, 위의 소견이 더 specific to PTE = LV RV 구분은 시행자가 잘 알겠지만, mitral valve(LV)가 tricuspid valve에 비해 더 위에 위치
• ECG (rt axis deviation, RV hypertrophy) = S1Q2T3 pattern : lead 1 deep s, lead 3 Q&T wave inversion(RV overload) = sinus tachycardia + T wave inversion(V1-4)
• D-dimer → CT, CT angiography → lung scan → venous us

• dyspnea +, but CXR only cardiomegaly → embolism 의심 → v/s stable 시 anticoagulant(LMWH) → IVC filter(출혈 위험성) → v/s unstable 시 tPA → embolectomy

• FiO2, PAO2

• FiO2 : 매 호흡에 포함된 산소의 퍼센트
• PAO2 : 폐포 산소 농도
• Five Times Rule : PAO2 = FIO2 x 5 (room air 20% - 100 / 40% - 200 / 60% - 300 / 80% - 400 / 100% - 500)
• capnography : CO2 of end of the expiration phase(EtCO2)
• pulse oximetry : 손가락으로 Saturation of Hb with O2 (92% 미만으로 떨어지면 arterial O2 content 급격히 감소)

• asthma, COPD
• Ventolin solution(nebulizer, SABA) : 기관지 평활근 이완 → 기도 확장
• 가래 있을 경우, mucomyst sol.(acetylcysteine)로 빼주기 (nebulizer로)
• PGI2(prostacycline) agonist 인 Treprostinil : 혈관 확장 및 폐 압력 낮춰 RV afterload 감소 효과, 약 중단 시 반동성 폐동맥 고혈압 발생하여 위험하니 절대 임의로 투여 중단하면 안 됨

• COPD sat target = 88-92%, CO2 retention 가능하니

• epiglottitis

• 응급, xray 상 thumb sign, 보이면 지체없이 intubation 해야함, 시간 놓치면 산소 줄 방법 없음
• sitting position, cortisol inj(염증 줄이기), epi nebulizer(염증 부위 혈관 수축, 부종 감소), mucomyst 로 가래 빼주기
• anti - 3세대 cepha, pipera-tazo, amikacin, metronidazole

• acute bronchitis
• 5일 이상 지속되는 기침(감기는 upper, 기관지염은 조금 더 아래인 bronchi에의 infection)

• 주로 self limited → 증상 완화용으로 기침, 가래약 정도만 줘도 됨

• common cold

• influenza A,B = oral oseltamivir(tamiflu), inhaled zanamivir, IV peramivir

• peramivir(페라미)2V + ns mix IV 로 주면 퇴원약 필요없음 / 주사 안맞으면 퇴원약 tamiflu PO

• pneumonia

• infection of alveolar portion of the lung
• cough, sputum, dyspnea, fever fatigue, pleuritic chest pain

• extrapul sx 으로 hematuria(Legionella), rash(Mycoplasma의 maculopapular rash or urticaria)도 가능

• Hemoptysis

• lt lung 인 경우, Fogarty catheter 포가티 카테터를 왼쪽으로 넣어 폐쇄 or ET tube 그대로 더 깊숙하게 넣으면 자연스럽게 rt bronchi 로 들어가 airway 확보됨 or lt lat decubitus 로 왼쪽이 아래로 가게 눕기
• rt = 짧고 수평에 가깝고 / lt = 길고 경사가 더 크다

• massive hemoptysis 기준 = 시간당 종이컵 반컵이상시(100ml), 한 번에 종이컵 한 컵 이상시(200ml)

• lung empyema (empyema, abscess 임상 양상은 유사)

• split pleural sign으로 visceral, parietal pleura 분리되고 그 안 pleural cavity 에 pus
• compression of involved lung

• regular, thin wall → early tube drainage, anti

• lung abscess

• lung parenchyme destruction으로 localized suppuration(폐 실질 내부에 위치)
• air - fluid level (아래는 농, 위에는 기관지 연결되어 공기 참)
• irregular, round, thick walled cavity → postural drainage, prolonged anti

Enhance lung CT : 혈관 보고 싶을 때, meta pleural effusion의심 시 - PTE, hemoptysis

Low dose chest CT - pneumonia, pleural effusion by onco D/dx
