Methylphenidate

• methylphenidate
• ADHD 치료제, DNRI(dopamine-NE reuptake inhibitor)
• 속방정(페니드, 지속시간 3-4시간), 서방정(콘서타, 메디키넷, 지속시간 8-12시간)
• 주의력과 집중력을 조절하는 dopamine, NE increase
• Sx = tachycardia, HTN, arrythmia, chest pain, tremor, myoclonus, seizure, headache, hyperthermia, agitation, hallucination, n/v
• Tx
  • hydration
  • charcoal
  • seizure 및 agitation : ativan
  • HTN : ativan, phentolamine(a blocker, 수축된 혈관 직접 이완), nitroprusside or nitroglycerine(혈관 확장제) / propranolol 단독 사용은 피할것(b block 시키면, 반동적으로 a adrenergic 심해져 혈관 더 수축해 혈압 폭등 가능)
  • tachycardia : ativan 5mg, esmolol 100mg 0.25V+NS100 mix
• 12시간 경과 관찰 후 tachycardia, BP, BT 정상화 되면 퇴원
