내탈장 및 폐쇄고리 장폐색 / Internal hernia and closed-loop obstruction

정의

내탈장은 복강 내 결손이나 공간을 통해 장관이 비정상적으로 들어가 생기는 폐쇄 양상이다. Closed-loop obstruction은 장의 한 구간이 두 지점에서 막혀 고립되는 형태로, 허혈·교액 위험이 높다.

임상 핵심

응급실 CT에서 가장 중요한 패턴은 국소적으로 뭉친 C/U-shaped small bowel loop와 mesenteric swirl이다. 2개의 transition point, converging vessels, vessel crowding이 보이면 closed-loop 가능성이 올라간다. Mesenteric edema, ascites, bowel wall hypoenhancement, pneumatosis, portal venous gas는 허혈/교액 red flag다.

진단

CT에서 clustered small bowel loops, C- 또는 U-shaped configuration, mesenteric swirl, 2 transition points를 찾는다. Reduced bowel wall enhancement 또는 marked wall thickening은 bowel ischemia를 시사한다.

치료

Closed-loop 또는 ischemia 의심 소견이 있으면 지연 없이 외과 협진 및 수술적 평가가 필요하다.
