어지럼 말초성-중추성 감별 / Peripheral vs central dizziness

정의

어지럼 감별은 BPPV, vestibular neuritis, Meniere disease 같은 말초성 원인과 stroke/hemorrhage/cerebellar infarction 같은 중추성 원인을 구분하는 응급 접근이다.

임상 핵심

말초성 어지럼은 빙글빙글 도는 양상이 흔하고, 중추성은 neurologic symptom, 두통, cerebellar infarction 가능성을 함께 본다. BPPV는 머리 움직임에 의해 악화되고 head trauma 이후에도 발생할 수 있다. Vestibular neuritis는 수시간-수일 지속되는 안진, abnormal head impulse, 정상 skew test가 기록되어 있다. 중추성 안진은 bidirectional 또는 vertical, 정상 head impulse, abnormal skew test가 단서다. 퇴원 시 ambulation well, neurologic symptom(side weakness/dysarthria) 없음 등을 기록한다.

진단

Dix-Hallpike test는 머리를 빠르게 젖혀 BPPV를 유발한다. Roll test는 supine 자세에서 머리를 좌우 90도로 빠르게 돌려 확인한다. HIT는 환자가 검사자 코를 보게 하고 머리를 좌우로 빠르게 돌려 corrective saccade를 본다. Peripheral nystagmus는 단일 방향 수평/사선, 시선 고정 시 감소하며 central nystagmus는 방향전환성/수직성 가능성이 있다.

치료

항구토제 metoclopramide, ondansetron, scopolamine patch, AH1 계열 dimenhydrinate/diphenhydramine/meclizine 등이 언급되어 있다.
