합병 담낭염 / Complicated cholecystitis

정의

합병 담낭염은 단순 급성 담낭염을 넘어 괴저, 벽 괴사, 천공 임박/국소 천공, 기종성 담낭염 등을 시사하는 상태다.

임상 핵심

담낭벽의 irregular enhancement가 심하거나 focal defect, wall discontinuity가 보이면 단순 급성 담낭염만으로 보지 않는다. Gangrenous cholecystitis, wall necrosis, impending 또는 contained perforation, emphysematous cholecystitis를 함께 감별한다.

진단

CT에서 decreased 또는 absent wall enhancement는 괴저/벽 괴사를 더 강하게 시사한다. Intraluminal membrane, wall defect, pericholecystic abscess가 있으면 gangrenous 또는 perforated cholecystitis 쪽으로 해석한다.

치료

이 자료는 구체적 치료 알고리즘을 제시하지 않는다. 다만 합병 담낭염 가능성이 있으면 영상 소견을 단순 담낭염보다 높은 위험도로 보고 임상적 긴급도를 올려야 한다.
