중심척수증후군 / Central cord syndrome

정의

중심척수증후군(CCS)은 불완전 경추 척수손상의 한 형태로, 전형적으로 상지 근력저하가 하지보다 심하고 sacral function은 비교적 보존된다. 고령, 기존 cervical spondylosis/stenosis, hyperextension trauma에서 흔하며 골절/탈구 없이도 발생할 수 있다.

임상 핵심

손 intrinsic muscle 및 finger function 저하가 두드러지고, sensory loss는 다양하며 burning dysesthesia나 hyperesthesia가 동반될 수 있다. 방광기능장애 또는 urinary retention이 가능하므로 bladder scan 및 필요 시 도뇨를 고려한다. 회복은 대체로 하지 → 방광 → proximal upper limb → hand function 순으로 진행되는 경향이 있다. 고령, 초기 motor deficit이 심한 경우, MRI상 hemorrhage나 긴 cord edema, 지속 compression/instability는 불량 예후 인자다.

진단

초기에는 ATLS 원칙에 따라 ABCDE, spinal motion restriction, rigid collar, log roll을 시행한다. 상세한 motor/sensory/reflex/sacral exam을 기록하고 ASIA/ISNCSCI score로 baseline을 남긴다. CT C-spine은 fracture/dislocation 평가의 1차 영상이다. MRI C-spine은 cord edema/hemorrhage, disc herniation, ligament injury, epidural hematoma, persistent compression 평가에 중요하다. Neurologic deficit 진행 여부는 serial exam으로 추적한다.

치료

골절/탈구, 명확한 instability, 심한 ongoing cord compression이 없고 deficit이 stable 또는 호전되면 immobilization, hemodynamic optimization, pain control, DVT prevention, bladder/bowel care, early rehabilitation을 시행한다. Fracture/dislocation, instability, acute disc herniation, epidural hematoma, severe stenosis로 인한 persistent cord compression, neurologic deterioration이 있으면 decompression/fixation을 고려한다. AO Spine/Global Spine J guideline은 acute SCI/CCS에서 24시간 이내 decompressive surgery를 옵션으로 고려할 수 있다고 권고한다. Acute traumatic SCI에서는 hypotension을 피하고, 기관 spine/neurosurgery protocol에 따라 MAP augmentation을 고려한다. 고용량 methylprednisolone은 routine standard of care로 보기 어렵고 감염, GI bleeding, hyperglycemia 위험 때문에 기관 protocol과 spine specialist 판단이 필요하다.

정의

Central cord syndrome은 incomplete cord syndrome의 한 형태로, 척수 중심부 손상에 따른 특징적 운동장애를 보인다.

임상 핵심

상지 근력저하가 하지 근력저하보다 더 심한 양상이 핵심이다. Sacral function은 비교적 잘 보존될 수 있다. 방광 기능장애가 동반될 수 있다.

진단

고령 환자, cervical spondylosis 또는 stenosis가 있는 환자에서 hyperextension trauma 후 발생하는 패턴을 의심한다. 외상 기전과 신경학적 진찰에서 상하지 근력 차이, 감각, 배뇨 기능을 확인한다.

치료

척수손상이 의심되면 경추 고정, 신경학적 악화 감시, 척추/신경외과 평가가 필요하다. 영상 평가를 통해 압박 병변과 불안정 손상 여부를 확인한다.
