부프로피온 중독 / Bupropion poisoning

정의

Bupropion 중독은 norepinephrine/dopamine reuptake 억제 약물의 과량 복용으로 경련과 심전도 이상을 유발할 수 있는 중독이다.

임상 핵심

• Minimal serotonergic activity를 가진 atypical antidepressant로 정리되어 있다.
• Raw 자료의 흡수 시간(regular 2시간, sustained release 3시간), 독성 가능량(>450 mg), 관찰 시간(24시간 이상)은 source-bound 전까지 raw-only 예시로 취급한다.
• Bupropion, 특히 sustained/extended-release 과량복용은 지연성 경련과 심전도 이상 가능성이 있어 관찰과 ECG monitoring 방향은 유지한다.

진단

• 흔한 부작용으로 tremor, dizziness, agitation, nausea, headache, dry mouth, constipation, anxiety, confusion, hallucination, psychosis 등이 언급된다.
• ECG는 sinus tachycardia가 대표적이며 wide QRS, QT prolongation도 가능하다.

치료

• 경련은 benzodiazepine 기반 치료를 우선 고려하되, lorazepam 등 특정 약제·용량은 프로토콜/출처 확인 전 확정하지 않는다.
• ECG monitoring을 시행한다.
• Activated charcoal은 selected early presentation에서 고려할 수 있으나 1시간 이내는 source-bound 전까지 일반 원칙 수준으로만 보존한다.
