발목 골절 신경혈관 진찰 / Ankle fracture neurovascular examination

정의

발목 골절 또는 골절-탈구에서 혈류와 말초신경 기능을 평가하고 기록하는 필수 신체진찰.

임상 핵심

Neurovascular exam은 반드시 기록하며, reduction 전후로 반복해야 한다. 혈관 평가는 DP pulse, PT pulse, capillary refill, foot color, temperature를 포함한다. 신경 및 운동 기능 변화는 골절 자체, 탈구, 부종, 정복 과정과 관련될 수 있어 시간 순서 기록이 중요하다.

진단

감각은 superficial peroneal nerve(dorsum of foot), deep peroneal nerve(1st web space), tibial nerve(plantar foot), sural nerve(lateral foot), saphenous nerve(medial foot)를 확인한다. 운동은 EHL great toe extension, FHL great toe flexion, tibialis anterior dorsiflexion, gastrocnemius/soleus plantarflexion을 확인한다. Pain out of proportion, passive toe stretch pain, progressive paresthesia, tense compartment는 compartment syndrome warning sign이다.

치료

혈관 저하나 진행성 신경학적 이상이 있으면 긴급 정복 및 정형외과 평가가 필요하다. 정복 후 호전 여부를 재평가해 문서화한다.
