급성 인두편도염 / Acute pharyngotonsillitis

정의

급성 인두염 또는 편도염으로, sore throat는 pharyngitis, 붉은 편도는 tonsillitis로 정리된다.

임상 핵심

Routine 항생제 투여 대상이 아니다. 항생제는 modified Centor score 3점 이상이거나 group A streptococcus rapid antigen test 양성일 때 고려한다. Penicillin allergy가 있으면 macrolide 계열을 대안으로 사용한다.

진단

증상과 인후/편도 소견을 확인한다. Modified Centor score와 S. pyogenes rapid Ag test 결과로 항생제 필요성을 판단한다.

치료

Amoxicillin 500 mg bid가 기본 선택지로 기록되어 있다. 대안으로 amoxicillin/clavulanate 고용량 tid가 언급된다. Penicillin allergy 시 azithromycin 500 mg qd 또는 clarithromycin 250 mg bid를 사용한다.
