넓은 QRS 대 좁은 QRS / Wide vs narrow QRS

정의

성인 ECG에서 narrow QRS는 QRS duration <120 ms, wide QRS는 ≥120 ms로 구분한다. 25 mm/s ECG 기준으로 120 ms는 작은 칸 3칸에 해당한다. 110–119 ms는 borderline 또는 mildly prolonged QRS로 보며 incomplete BBB나 nonspecific intraventricular conduction delay 등으로 표현될 수 있다.

임상 핵심

Narrow QRS는 대개 His-Purkinje system을 통한 정상 심실전도를 시사한다. Wide QRS는 ventricular origin, aberrant conduction, bundle branch block, pre-excitation, pacing, metabolic-toxic cause 등을 고려한다. 빈맥에서 QRS 폭은 narrow-complex tachycardia와 wide-complex tachycardia 접근을 나누는 핵심 기준이다.

진단

Regular tachycardia에서 QRS <120 ms이면 AVNRT, AVRT, atrial tachycardia, atrial flutter 등 narrow-complex tachycardia를 우선 고려한다. QRS ≥120 ms이면 wide-complex tachycardia로 분류한다. 구조적 심질환, 고령, AV dissociation, capture beat, fusion beat가 있으면 ventricular tachycardia 가능성이 높다.

치료

응급실/ACLS 접근에서는 wide-complex tachycardia를 원칙적으로 VT로 간주하고 안전하게 접근한다. 혈역학적 안정성 여부에 따라 ACLS tachycardia algorithm에 따라 처치한다.
