정의

Synchronized cardioversion은 QRS에 맞춰 shock을 전달하고, unsynchronized defibrillation은 동기화 없이 즉시 고에너지 shock을 전달하는 전기치료 방식이다.

임상 핵심

• 핵심 비교 축은 pulse 유무, rhythm의 sync 가능성, shock 지연 위험이다.
• pulse가 없는 환자는 unsynchronized defibrillation 대상이다.
• pulse가 있고 불안정한 빈맥에서 sync가 가능하면 synchronized cardioversion을 선택한다.
• polymorphic VT/torsades처럼 sync가 불안정하면 unsynchronized defibrillation을 시행한다.

진단

• 먼저 pulse를 확인한다.
• 다음으로 QRS sync marker가 안정적으로 잡히는지 확인한다.
• AF with WPW는 불규칙 wide rhythm이어도 sync 가능한 경우 synchronized cardioversion 대상이다.
• torsades/polymorphic VT는 sync가 어려운 대표 상황이다.

치료

• Synchronized cardioversion: unstable tachycardia with pulse and syncable QRS.
• Unsynchronized defibrillation: pulseless rhythm, polymorphic/chaotic rhythm, 또는 sync marker가 잡히지 않아 shock 지연이 위험한 상황.
