간경변성 vs 비간경변성 위장관 출혈의 배양검사와 항생제 / Cirrhotic vs non-cirrhotic GI bleeding culture and antibiotics

정의

위장관 출혈 환자에서 간경변/문맥고혈압 배경 유무에 따라 blood culture와 예방적 항생제의 필요성을 구분하는 비교 개념이다.

임상 핵심

간경변/문맥고혈압 + UGIB 의심이면 출혈원이 정맥류로 확정되지 않아도 항생제 prophylaxis를 시작한다. 간경변이 없는 일반 non-variceal GI bleeding에서는 fever나 sepsis source가 없으면 blood culture와 항생제 prophylaxis가 루틴이 아니다. 애매한 경우라도 LC 환자의 GI bleeding이면 cirrhotic GI bleed로 보아 항생제 쪽으로 기운다.

진단

Cirrhotic GI bleeding: 감염/SBP 의심 시 blood culture 2쌍과 ascitic fluid culture를 동시에 또는 가능한 즉시 채취한다. Non-cirrhotic GI bleeding: fever, rigors, septic physiology, cholangitis, ischemic/infectious colitis, perforation, aspiration pneumonia, line infection 등 감염 의심이 있을 때만 culture를 시행한다.

치료

Cirrhotic/portal HTN 배경: culture를 기다리지 말고 항생제 예방 투여. Non-cirrhotic 단순 GI bleed: bleeding workup, resuscitation, endoscopy pathway가 중심이며 항생제는 루틴이 아니다.
